Help for FORGOT PASSWORD:
It is all too common to forget
one's password. You are not alone with this problem. This menu is designed
to help you get logged on again. If we can identify you from your entries; problem
solved. If not, we will email you with further instructions.
Help for FIRST NAME:
"First Name" is your first name
only on a single line. The name may contain only alphabetical characters or an
apostrophe; e.g. a last name of O'Hare is acceptable. The entry may not exceed
29 characters.
Help for LAST NAME:
"Last Name" is your last name only
on a single line. The name may contain only alphabetical characters or an apostrophe;
e.g. a last name of O'Hare is acceptable. The entry may not exceed 29 characters.
Help for EMAIL:
The "Email" entry is the address to
where our email to you will be sent.
Help for ZIP CODE:
The "Zip Code" must be five (5)
numerical characters. It is used internally by the program to help identify you
when you make a typo in your name or password.
Help for REQUEST PASSWORD BY EMAIL:
In order to process
your request, one must click the "Request Your Password" button. It is OK to leave
the "Request Password by Email" page by any other method comfortable to you if you do not
want to search for your password.
